DEPOSIT AND PAYMENTS: A 50% deposit will be due upon submission of this contract. This deposit will be applied to the last shifts of care. Payments are due every week. In the event that you choose not to use my services, the deposit will not be refunded. Payment can be made by Venmo, Zelle, or Paypal. TRAVEL FEES: If your location is out of my service area, a small travel fee will be applied. OVERNIGHT SERVICES: Overnight postpartum care is not available at this time with Nurture and Empower. If requested, a referral to another postpartum doula will be made to support nighttime care. SERVICES AND RESPONSIBILITIES: I agree to provide non-medical physical, emotional and informational support after the birth of your baby. This can include help with self-care, recovery, postpartum comfort measures, newborn care, parenting information, sleep support and tips, and providing assistance with breastfeeding and/or bottle feeding your baby. Practical support will include preparing snacks and light meals, getting frequent drinks, doing dishes and laundry. The tasks I perform each day will depend on the priorities discussed by all parties before beginning services, and on your particular needs that day. I do not and am not qualified to diagnose any medical conditions for mother or baby but will refer you to an appropriate health care provider if concerns arise. I do not do major housecleaning tasks, such as mopping, washing windows, or yard work. SPECIAL CIRCUMSTANCES & REFUND POLICY: Prior to signing this agreement, I will disclose any potential scheduling difficulties I anticipate which may interfere with services and work with you to ensure you still receive the care you require. In the event of illness, emergencies, or other unexpected and/or extraordinary circumstances, for either party, every effort will be made to reschedule services at another time. If any payment for that day has already been made, it will be applied to future services. ILLNESSES: The following pertains to sicknesses or illnesses and is intended to ensure my own and your family’s health and safety while providing services. I will not provide services in any client's home while experiencing symptoms. I reserve the right to postpone services until I have been symptom-free for 24 hours. I will not provide services in any client home that is experiencing symptoms due to the potential for exposure to my other clients and babies. In the event that I am unable to attend shifts due to an illness those missed hours can be rescheduled. In the event that you cancel a shift due to you or a member of your household is showing symptoms those hours can be rescheduled. No refunds will be given for hours missed should you decline to reschedule them. I reserve the right to offer any kind of refund at my discretion depending on mitigating circumstances.
